You will be asked to fill out a confidential intake form at your first visit. Please arrive a few minutes early.
Payment is due at the time of service. Cash, check or credit cards accepted.
Cancellation policy: Please give 24 hours’ notice to avoid a fee.
Insurance: Please have a prescription from a doctor that includes ICD-10 diagnosis code(s) and amount of visits authorized. If you have been involved in a car accident or on-the-job injury, you will need a claim number and the contact info for the insurance company.
In-Network: L&I (on the job) and PIP (auto accidents).
